Statement of Services Sample Forms:
The Colorado Colorectal Screening Program's patient navigation service helps patients overcome barriers to screening and care for colorectal cancer. This service includes support throughout the screening process as well as post-screening guidance, if follow-up is needed.
A link is included below for the Colorectal Cancer Patient Navigation Guide, a colorectal screening resource guide for patient navigators, case managers, outreach workers and medical system guides. We will update this manual regularly to keep navigators informed of changes in the Colorado Colorectal Screening Program and to provide additional resources to anyone who is helping guide patients through colorectal screening.

Cultural Competency
America is becoming much more culturally diverse. Physicians and other health care providers who learn about cultural differences can communicate more effectively with people from different cultural backgrounds, and they can better convey to them the value of cancer screening tests in preventing colorectal cancer.
The Colorado Colorectal Screening Program and the University of Colorado School of Medicine recommend, as a starting point, the free online training tool A Family Physician’s Practical Guide to Culturally Competent Care. Physicians who complete the modules are eligible to receive 2 COPIC ERS points and up to 9 CME credits.
