For Accepted Fellows
Congratulations on being selected as a Student Cancer Research Fellow at the University of Colorado Cancer Center. We hope that your experience is rewarding. We look forward to having you with us for the summer. Before you start your fellowship, you will need to:
- Complete and Return your Agreement Form
- Complete and Return Human Resources Forms and Background Check Form
- Complete Health and Safety Training
1. Complete your Agreement Form
We have emailed you a Student Agreement Form. Please complete, sign and send us this form within three days of receiving it. You can fax it to the UCCC Grants and Education Office, then mail the original copy to us. Our contact information is on the right.
2. Complete Human Resource Forms and Background Check Form
Within two weeks of receiving your Agreement Form by email, you must complete and return the originals of the following Human Resource Forms to the UCCC Grants and Education Office. Please mail all forms to Jill Penafiel (contact info. on the right)
- Personal Information Worksheet W-4 for Tax Withholding and Instructions for W-4
- I-9 Employment Eligibility Verification Form To complete the I-9 form, you must bring original documents verifying your eligibility to work in the United States to your orientation session. List A, B and C found on the form will indicate which forms you will need. Until you present the proper forms, you will not be paid. If you plan to start your fellowship before Friday, June 5, 2008, you must make special arrangements to bring your Employment Eligibility Verification documents to the Grants and Education office before your start date. Please contact Jill Penafiel to arrange this meeting.
- Direct Deposit Form University of Colorado employees are paid through direct deposit into a bank account. Please complete this form and attach a voided check.
- Confidentiality Form You must sign a confidentiality statement for University of Colorado Denver. Please read it carefully, sign it, and return it with the Human Resources Forms.
- Background Check Form All employees at the University of Colorado (including students) are required to undergo a background check.
3. Complete Health and Safety Training
You will be working in a laboratory setting during your fellowship, and we want you to be as safe as possible. You must complete several online training classes before your first day of work.
You’ll need an employee ID number to take the online courses. We will send it by email once we receive your completed Human Resources forms and the approved background check. Your employee ID is both your username and your password.
If you are working at a non-CU location, please check with your preceptor about other training that may be required.
- Hazardous Waste Generators Class
- HIPAA Compliance Training. The Health Insurance Portability and Accountability Act of 1996 helps ensure that confidential patient information stays private. You are required to take this online HIPAA course.
- Other Safety Training. Some projects may require additional safety training, like blood-borne pathogens, radiation safety, etc. Your preceptor will know if you need to take these classes, and help you make arrangements to take the course on campus if it isn’t available online.
